Alabama Medical Cannabis Card Registration Guide

If you have received a certification from a registered certifying physician and are applying for a medical cannabis card for the first time, you must complete the New Patient Registration process. This registration is handled through the official Alabama Medical Cannabis Commission (AMCC) licensing portal.

Follow the step-by-step instructions below to ensure your application is submitted correctly and  processed without delay.

Step 1: Primary Account Creation

Before submitting an application for a medical cannabis card, you must create an account on the AMCC Portal. Once you’ve created your primary account and completed the email verification steps, proceed to Step 2 for instructions on starting your application.

  1. Access the Portal: Navigate to the AMCC Portal.
  2. Click the “Register” button in the upper right-hand corner of the login page.
  3. Complete all required fields. Note: Physicians, patients, and caregivers must create an “Individual” account.
  4. After entering all required information, you must (1) confirm that you have read and accept the Terms and Conditions; and (2) pass the reCAPTCHA test to confirm that you are not a robot.
  5. Click “Register” to complete
  6. A verification email will be sent to the email address provided in the registration information. You must open the email and verify your registration. If you do not receive a verification email, you can return to the new user registration page and request that the email be resent. If you continue not to receive the email, please check your spam and junk folders.
  7. After completing the verification email, you can login to the AMCC Portal using your registered email and password and create an application. Instructions for creating an application by user type (Physician, Patient, Caregiver) are available on the AMCC website.

Step 2: Access and Initiation

Once you’ve completed your account set up and email verification in Step 1, you may start your new application for a medical cannabis patient card.

  1. Access the Portal: Navigate to the AMCC Portal.
  2. Login: Use the email address and password you created during account setup.
  3. Start Application: Click the + CREATE NEW APPLICATION button on the left-hand panel of your account page.
  4. Select Type: Choose “Patient” from the “I am a” dropdown.
  5. Choose Registration: Select “New Patient Registration” and click CREATE APPLICATION.

Step 3: Personal and Contact Information

  1. General Info: Complete all required items on the “General Information” tab.
    • Note: This information MUST match exactly what your certifying physician entered on your Electronic Physician Certification.
  2. Minor Patients: If the applicant is under 19 or has a court-appointed legal guardian, parent/guardian info must be provided.
  3. Contact Info: Click SAVE & NEXT and complete the “Contact Information” tab.

Step 4: Physician Certification and Caregivers

On the “Certifying Physician/Condition Information” tab, click VIEW AVAILABLE CERTIFICATIONS to select your valid physician’s certification.

  • Prepopulated Data: This information is pulled directly from your doctor’s entry. You cannot change it; if it is incorrect, you must contact your physician to update it.
  • Caregiver Requirements: Minor patients or those requiring assistance must select “Yes” for the caregiver question. The caregiver info must match what was entered on your physician certification.

Step 5: Attestation, Documents, and Payment

  1. Attestation: Sign and date the “Attestation” tab.
  2. Upload Documents: Use the UPLOAD button on the “Documents” tab to submit required files.
  3. Payment: Select your payment method (Visa, MasterCard, Discover, or ACH/e-check).
    • Digital Card Fee: $40.00.
    • Optional Physical Card: $10.00 additional.
    • Replacement Card: $25.00.

Step 6: Review and Submission

Before submitting, you will be directed to the “Review” tab.

  • Fix Errors: Any items marked with a X must be corrected by returning to the corresponding tab.
  • Final Step: Click PAY & SUBMIT at the bottom of the page.

Please keep in mind that if you find errors in the prepopulated data on your certification tab, you must contact your certifying physician directly, as they are the only ones authorized to update your official Electronic Physician Certification.

What happens after I submit my application?

You will receive an email notifying you that your application was submitted successfully and is pending review. A second email will follow to notify you if it is approved, denied, or returned for correction.

Where do I find my medical cannabis card once approved?

Your card will appear on your “License Dashboard”. You can download and print it by clicking PRINT DIGITAL CARD.

What should I do if my doctor entered my name incorrectly?

Because physician certifications prepopulate and are uneditable by the patient, you must contact your certifying physician to have them update the electronic certification before you can proceed.

How long is my Alabama medical cannabis card valid?

A registered patient’s card is valid for exactly one year from the date of issuance.

Can I use a credit card for the state fee?

Yes. The AMCC portal accepts Visa, MasterCard, and Discover, as well as ACH/e-checks.

Do I have to pay for a physical card?

The digital version is included in the $40 registration fee. If you would like a physical (hard) card, there is an additional $10 initial fee.

Does my caregiver need to be on my physician’s certification?

Yes. If you plan to use a caregiver, the information you enter in your registration must match the caregiver information your doctor entered on your certification.

What if I missed a required document in my application?

The system will direct you to the “Review” tab before submission. Any missing required items will be marked with a red X so you can correct them before paying.
