How to Obtain Your Certification

Per the AMCC Portal Instructions, your journey to wellness follows a strict three-step legal framework:

  1. The Clinical Evaluation: You must have an in-person physical exam with one of the physicians listed above. They will diagnose or confirm a qualifying condition.
  2. The Electronic Certification: Once approved, your doctor enters your data into the AMCC Patient Registry. Important: Ensure the doctor uses your legal name exactly as it appears on your ID.
  3. The Patient Application: After your doctor completes their part, you must log into the AMCC Portal to complete your registration, sign the attestations, and pay the state fee.
